- YOUR NAME
- YOUR HOME ADDRESS
- POSTCODE
- YOUR HP NUMBER
- YOUR EMAIL
- WHAT PRODUCT YOUR BUY. Example: T-shirt I Luv Band Malaya
- TOTAL MONEY THAT YOU BANK IN. Example: RM29.50 ( this total include Pos Express Cost)
- SIZE T-SHIRT (If your buying t-shirt ONLY)
